Office Record Review Overview
All diplomates participating in Maintenance of Certification are required to complete the Office Record Review (ORR). The ORR is a web-based self review of clinical practice utilizing modules that encompass the standards of care and practice patterns related to different diagnoses (ORR modules).
Each ORR module is an ophthalmic diagnosis with a specific definition and timeframe requirement, e.g., Cataract (surgical): Patient with cataract who has had cataract extraction by you on one or both eyes, seen by you at least once in the past 12 months. Completing the ORR
The ORR is available online, on-demand to eligible diplomates via the Online MOC Center. To complete the ORR, diplomates are required to select three ORR modules and review a series of five of their patient records for each. Once a diplomate registers for the ORR, they have 30 days to complete it.
ORR module selection should be based on a diplomate's clinical practice because for each of the three ORR modules chosen, diplomates will select five records of patients they have seen with that diagnosis within the timeframe specified. To register for each module, the diplomate is required to provide the following information about each patient record they will review:
- Year of birth, initial visit date, most-recent visit date, and a unique record identifier that will enable the diplomate to identify the records selected. Diplomates should use an internal chart referencing number, this identifier is for the diplomate’s use in identifying the patient records that will be used for the review.
- Attestation of direct patient care responsibility for each patient record selected for the review. Please note that direct patient care is required. If a diplomate is in a group-partnership or teaching position, exclusive care of the patient is not mandatory for selection of patient charts for the ORR, but he/she must be actively involved in the patient's care.
Diplomates will receive immediate feedback upon completion of the ORR.

Timeframe for ORR
ORR must be completed once during the MOC cycle. The MOC process is designed to be completed over the 10 year certification cycle with each component completed in a specific timeframe. Ultimately, all diplomates will complete the ORR during years 3-4 of their 10 year MOC cycle. However, during the transition to maintenance of certification, the components and their required timeframe will be different according to the year a diplomate was most recently certified or recertified.
