Instructions for Completing the 2007 Written Qualifying Examination Application

Overview Candidates who wish to be considered for the 2007 Written Qualifying Examination must complete the online application by August 1, 2006. Submission of all supporting information must be received in the Board office postmark or courier-dated by September 1, 2006.

The test will be given on Tuesday, March 27, 2007 at approximately 300 nationally distributed Prometric test centers. The 2007 Written Qualifying Examination will apply to a time-limited certificate valid for 10 years.

Please note: The following information is an overview of what to expect in each of the five sections of the online WQE application. These instructions do not outline the specific requirements necessary to obtain Board Certification. To review the requirements for obtaining Board certification, please read the 2006 Board Certification Brochure.

There are five steps to completing the online application for the March 27, 2007 WQE:

Step 1. Contact Information You will provide your most current contact information, including name, current address, telephone number, email address, and publication address.

Step 2. Personal Details You will provide detailed personal information including social security number and date of birth. (Our SSL will ensure that your information is secure at all times.) You will also submit information about your medical school, ECFMG information (if applicable) and your current license to practice medicine.

Important: Submitting Medical License Information
Submitting a Photocopy of your Current Renewal Registration to Practice Medicine
The ABO requires you to submit a photocopy of your valid and unrestricted license to practice medicine in the United States, its territories or in a Province of Canada in order to complete your application. Do not send your original license certificate; rather, submit a copy of your renewal registration which shows a current expiration date. The ABO will not accept training or educational licenses.

At the time of application, and at all times thereafter, all applicants must promptly inform the Board in writing if their license to practice medicine in the United States, any of its territories, or Canada has previously been surrendered or not renewed upon its expiration or if the candidate's practice of medicine, or license to practice medicine in any of those locations, has previously been revoked, restricted, conditioned, suspended, limited, qualified, or subject to terms of probation.

If you are unable to submit this license upon completion of the online application, you may apply for a license proviso until January 1, 2007. Please indicate if you are unable to provide license information at the time of application in section X.


Submitting a Photocopy of your ECFMG Certificate (if applicable)
Your must submit a photocopy of your ECFMG certificate if your undergraduate medical education was pursued outside the United States or Canada.

Step 3. Training Information:
This section asks you to provide or review your post-graduate training. The American Board of Ophthalmology tracks residency training and may have received verification of your progression through post-graduate education from your ophthalmology residency program. You will not be able to modify any information provided from outside sources, however, if there is information about your training that is missing you must provide the missing information in the appropriate fields.

Once you complete all five sections of the application and submit it electronically to the Board, the Board office will contact your ophthalmology residency to verify that you completed an accredited PG-1 year (minimum of six months of this year in direct patient care) and satisfactorily completed an entire graduated residency training program in ophthalmology of at least 36 months’ duration (PGY-4 or beyond). The program must verify the institution, location and exact dates of the PG-1 year and the dates of the ophthalmology residency reported on your application. The Board office will contact the Program Chair and Director for this statement, which must be returned to the Board office with a postmark date of no later than October 1, 2006.

After submission of the satisfactory completion form, you will once again be notified of your application status.

Step 4. Terms and Conditions
All candidates are required to accept the terms and conditions for participation in the Board certification process and examination procedures.

Step 5. Payment
The fee for the 2007 WQE is $1500. The ABO accepts only Visa and MasterCard. Upon successful completion of the Written Qualifying Examination, an additional fee is payable for admittance to the Oral Examination.

Application Completion
Upon submission of the online application and payment, candidates must submit appropriate supporting information, including a photocopy of your current renewal registration to practice medicine, which shows a current expiration date and if applicable, a photocopy of your ECFMG certificate (required if undergraduate medical education was pursued outside the United States or Canada.). Supporting documentation must be received by the Board office with a US postmark date or courier date of September 1, 2006.

All registered candidates will receive a mailing in late January 2007 with general WQE information and a Scheduling Permit that will provide specific instructions for scheduling an appointment at a Prometric test center (www.prometric.com). Candidates should schedule immediately upon receipt of the permit. The availability of approximately 300 testing centers should enable candidates to take the WQE in a location close to their home or practice. Click here for a list of available testing centers. Every effort will be made to ensure a requested test site is honored; however, candidates who do not schedule in a timely fashion may not be able to schedule an appointment at their desired testing center.
